Most Testosterone Booster Supplements Don’t Survive Their Own Clinical Trials

Walk down any supplement aisle and you’ll find dozens of bottles promising to boost testosterone naturally. A 2020 review in the World Journal of Men’s Health tested 50 of the best-selling testosterone boosters against the actual clinical evidence for their ingredients. Only 24.8 percent of the studied ingredients had research showing a positive effect on testosterone. More than half had no supporting studies at all.

That gap between marketing and evidence is the story of the testosterone booster industry. But it isn’t the whole story. A handful of ingredients have held up in randomized trials, and understanding why some work while most don’t reveals a lot about how testosterone actually responds to what you put in your body.

Why testosterone became a wellness obsession

Total testosterone in American men has been declining for decades, independent of age. A widely cited Journal of Clinical Endocrinology and Metabolism analysis of Massachusetts men found average levels dropped roughly 1 percent per year between 1987 and 2004, a trend researchers linked to rising obesity rates rather than aging alone.

That decline, combined with symptoms like fatigue, reduced libido, and difficulty building muscle, has turned testosterone into shorthand for vitality itself. Supplement companies have responded with a flood of products, most built around a familiar list: fenugreek, D-aspartic acid, tribulus terrestris, zinc, and ashwagandha. The ingredients repeat across brands, but the evidence behind each one varies enormously.

The ingredient with the strongest track record

Ashwagandha has more consistent clinical support than almost anything else in this category. In a trial published in the American Journal of Men’s Health, resistance-trained men who took 600 mg of ashwagandha root extract daily for eight weeks saw testosterone rise by roughly 14.7 percent more than the placebo group, alongside greater strength gains and faster recovery.

The proposed mechanism isn’t a direct hormonal push. Ashwagandha appears to lower cortisol, the stress hormone that competes with testosterone production in a shared pathway, according to lead researcher Adrian Lopresti. Chronically elevated cortisol suppresses luteinizing hormone signaling to the testes, so bringing cortisol down can indirectly free up testosterone synthesis.

Shilajit, a mineral-rich substance harvested from Himalayan rock, has smaller but notable support too. A 90-day trial in Andrologia found purified shilajit raised total testosterone by about 23.5 percent in healthy men aged 45 to 55 compared with placebo. Researchers there pointed to fulvic acid and trace minerals like selenium and zinc as likely contributors, though the study was small and hasn’t been widely replicated at scale. Where the evidence quietly falls apart

D-aspartic acid is the clearest example of a supplement built on one promising study that later failed to replicate. The original 2009 trial in Reproductive Biology and Endocrinology reported a 42 percent testosterone increase in sedentary men after 12 days. But a 2017 follow-up in the Journal of the International Society of Sports Nutrition, conducted in resistance-trained men, found no change in testosterone at all, and some evidence that higher doses actually suppressed it.

The difference likely comes down to baseline hormone status. D-aspartic acid appears to help only in men who are already testosterone-deficient, not in healthy, trained lifters who make up most of the supplement’s customer base. That distinction rarely makes it onto a label.

Tribulus terrestris follows a similar pattern. Despite being one of the most common ingredients in commercial blends, a systematic review in the Journal of Dietary Supplements found no consistent evidence it raises testosterone in humans, even though it reliably does in some animal studies. Zinc and vitamin D fall into a narrower category: they help only when you’re deficient. A study in Nutrition found zinc supplementation restored testosterone in men with confirmed zinc deficiency, but had no effect on men with normal levels to begin with. What actually moves the needle more than any pill

Nir Barzilai, director of the Institute for Aging Research at Albert Einstein College of Medicine, has noted in interviews that lifestyle factors dwarf supplement effects when it comes to hormone levels. Sleep is the clearest example. A study published in JAMA found that restricting healthy young men to five hours of sleep per night for one week reduced daytime testosterone levels by 10 to 15 percent, an effect comparable to aging 10 to 15 years.

Body fat matters just as much. Adipose tissue contains aromatase, an enzyme that converts testosterone into estrogen, so higher body fat percentages are consistently associated with lower free testosterone independent of age. A persistent myth worth retiring

One claim that circulates constantly in gym forums is that creatine spikes testosterone or its more potent derivative, DHT. The evidence doesn’t support it. For men with normal levels looking for marginal support, ashwagandha and shilajit have the most defensible research behind them, largely because they address stress and micronutrient status rather than attempting to force hormone production directly.
