Almost 90 Percent of Adults Miss the Daily Choline Target Their Brain Needs

Roughly 90 percent of adults in the United States do not consume enough choline, according to national dietary survey data, even though the nutrient sits at the center of nearly every process that keeps the brain running.

A new narrative review published in Nutrition Research makes the case that choline deserves far more attention than it gets. The authors describe it as a nexus nutrient, a hub that connects to folate, vitamin B12, and omega-3 fatty acids in ways that determine how well the brain builds cell membranes, produces neurotransmitters, and regulates gene expression over a lifetime.

What Choline Actually Does Inside the Brain

Choline has two jobs that matter most for cognition. It is the direct precursor to acetylcholine, the neurotransmitter responsible for memory formation, muscle signaling, and attention, and low acetylcholine activity is one of the hallmark features researchers have documented in Alzheimer’s disease.

It is also the raw material for phosphatidylcholine, the phospholipid that makes up a large share of every neuron’s cell membrane. Steven Zeisel, a nutrition researcher at the University of North Carolina who has studied choline metabolism for decades, has long argued that inadequate choline during critical developmental windows can permanently alter how the hippocampus, the brain’s memory center, is wired.

That is why choline intake during pregnancy has become a focus of its own research field, separate from the cognitive questions facing adults later in life. Cell membranes throughout the body are rebuilt constantly, and the brain, which is roughly 60 percent fat by dry weight, depends on a steady supply of phosphatidylcholine to keep that turnover running smoothly.

A Nutrient That Depends on Other Nutrients

The review’s central argument is that choline rarely acts alone. It participates in one-carbon metabolism, the same biochemical pathway that relies on folate and vitamin B12 to generate the methyl groups cells use to build DNA, repair tissue, and regulate which genes get switched on or off.

Choline can act as a backup methyl donor when folate is scarce, and folate can partially compensate when choline runs low. But the two nutrients are not fully interchangeable, and a shortfall in both at once appears to compound the damage rather than simply add to it.

This is part of why a methylated multivitamin that pairs active B vitamins with adequate choline intake makes more physiological sense than treating any single nutrient as a standalone fix. Genetic variation also plays a role, since a common polymorphism in the gene that helps the body synthesize choline internally leaves some people needing significantly more from food than others.

Where Adults Are Falling Short

The Institute of Medicine set an adequate intake of 550 milligrams per day for adult men and 425 milligrams per day for adult women. National survey data cited in the review shows the average American adult takes in closer to 300 to 400 milligrams, leaving a meaningful gap for most of the population.

Eggs are the most concentrated everyday source, with a single yolk supplying roughly 147 milligrams. Beef liver, salmon, and soybeans round out the list of foods that reliably move the needle, but choline is genuinely difficult to get in quantity from plant-forward diets alone.

Vegans and strict vegetarians face the steepest climb, since the richest sources are almost all animal products. Older adults are also at higher risk, both because appetite and food variety tend to decline with age and because absorption efficiency for several B vitamins that work alongside choline drops over time.

An earlier analysis of the Framingham Offspring cohort found that adults in the highest tier of dietary choline intake scored measurably better on verbal memory tests than those in the lowest tier, and showed less white matter damage on brain imaging. The association held even after adjusting for age, education, and other dietary factors.

Where Supplemental Choline Fits

Because whole-food choline intake is hard to hit consistently, supplemental forms like citicoline, also called CDP-choline, and alpha-GPC have become common ingredients in nootropic stacks. The TMAO Question

Choline’s reputation took a hit in 2011 when a study in Nature linked high dietary choline and carnitine to elevated trimethylamine N-oxide, or TMAO, a gut-bacteria byproduct associated with cardiovascular risk in some observational cohorts. The finding got wide coverage and left some readers wary of choline-rich foods and supplements altogether.

The newer review addresses this directly, noting that TMAO production depends heavily on individual gut microbiome composition rather than choline dose alone, and that the cardiovascular signal has been inconsistent across subsequent studies. Cutting choline intake to avoid a still-unsettled cardiovascular concern risks trading a well-documented cognitive benefit for a theoretical one.

Choline and Physical Performance

Endurance athletes have their own reason to pay attention to choline status. Blood choline levels measurably drop during long endurance events like marathons and triathlons, and some researchers have proposed that this decline contributes to the fatigue and slowed reaction time athletes report late in a race.

Trials testing choline supplementation before endurance events have produced mixed results, with some showing modest improvements in time-to-exhaustion and others showing no measurable performance benefit. The review frames this as an area where the mechanism is plausible but the applied evidence has not caught up yet, a common gap in nutrition science.

For anyone building a broader supplement routine around training and recovery, choline sits alongside creatine as a nutrient with strong mechanistic support that is still working out its exact performance dosing. Both are worth understanding on their own terms rather than treating either as a shortcut.

What This Means in Practice

The practical takeaway from the review is not that everyone needs a choline supplement. It is that choline deserves the same attention people already give protein or omega-3 fatty acids when they think about long-term brain health.

Two eggs at breakfast, a serving of salmon or beef a few times a week, and awareness of how choline interacts with folate and B12 status will cover most adults without any supplementation at all. For those relying on nootropic stacks that already include alpha-GPC or citicoline, the nutrient is likely already part of the routine, whether it was chosen deliberately or not.

Brain health research keeps circling back to the same conclusion: no single nutrient works in isolation, and choline’s real value shows up in how well it collaborates with everything else on the plate.
